Patients are responsible to pay for all fees due at time of service.

We accept as payment:  Cash, Check, Visa and Mastercard.

If possible, please download and fill out these three forms before you come to the office for your first visit.

Financial Policy

Charges for professional services are due at the time dental treatment is provided. We make every effort possible to provide you with dental treatment that fits your timetable is affordable and does not compromise in the quality of care or materials prescribed by the doctor. Our staff will be hapy to assist you in our private consultation area to review treatment options, procedures priorities, and payment arrangements.


Dental Insurance


Your insurance can be a helpful supplement towards your dental investment depending on your plans' benefit options. Be assured that we will use all necessary resources to ensure that you receive the maximum benefit under your particular plan.  Professional services that are rendered to you are fully your financial responsibility regardless of insurance coverage. If you carry dental insurance, it will be your responsibility to pay your co-payment and deductible at the time dental services are rendered. Our office will contact your insurance company initially for a brief estimate of benefits and levels of coverage. This estimated portion your insurance compay may pay is just an "estimate" and we cannot guarantee what, if any, your insurance covers towards your dental treatment. This service is a courtesy and cannot be binding due to insurance estimates under each plan. Our office will bill your insurance company as a "courtesy" and if the insurance company has not paid within 45 days of the charge, the full balance will be due immediately. We cannot guarantee what, if any, your insurance will pay towards your dental treatment.

Scheduling of Appointments

Our Business Assistants will offer your appointment options for your dental needs. Some treatment may require several visits and pre-planning. Once these visits are scheduled, at least 48 hours notice (not including weekends when the office is closed) is required in the event you need to change an appointment in order to avoid a rescheduling/broken appointment fee.  This gives us the opportunity to offer your appointment to a patient in need (dental emergencies).  We appreciate this courtesy because that emergency patient  waiting to schedule could be you in the future.  Please ask us about our priority call list for appointment availability.

Financial Options

Forms of Payment:
In order to facilitate access to the very best healthcare possible, you may choose form any of the following (including any combination thereof): Cash, Visa, MasterCard, American Express, Discover, Personal Check, Debit Cards, or Care Credit Financing.

As a service to our patients, we are pleased to offer the CareCredit Card. CareCredit lets you begin treatment immediately, then pay for it over time with low monthly payments that are easy to fit into your monthly budget.

CareCredit offers a full range of No Interest and Extended Payment Plans for treatment fees from $1 to over $25,000.

No Interest Payments Plans:
  • 3, 6, and 12 months plan options
  • No interest if balance is paid withing the specified time period
  • Low minimum monthly payments

Extended Payment Plans:
  • 24, 36, 48, and 60 month plan options
  • For procedures fees from $1,000 to over $25,000
  • 11.9% interest rate (check for current rates)
  • Monthly payments as low as $47 for a $1,000 fee balance

Senior Discount

For our cash patients 65 and over we will offer a senior courtesy discount on dental treatment paid in full at the time of service.

Prepayment Discount


Ask our financial business assistans about pre-paying for your next visit. A discount will be applied for advance payments depending on procedures appointed.

New Patients Experience

Your first visit after choosing our office for your dental services will involve a comprehensive exam and diagnosis of existing issues. At this appointment, which generally takes from one to one and a half hours, we will review your medical and dental histories and discuss any concerns you may have. We will perform a thorough examination of all of your soft tissues and an oral cancer screening. We will also take any x-rays needed for diagnosis and treatment. Finally, we will examine each tooth for decay or defective restorations.

A cleaning is not always performed at this first visit. Experience has shown us that every patient's needs are different and that we cannot anticipate what type of cleaning a patient needs or how much time is required without completing a thorough examination first.  Dr. Miller and his hygienists believe that preventative care is vital to long-term dental health and that, fo the sake of thoroughness, it should be therefore given all the time necessary.

Information Needed - from the patient


In scheduling your first appointment and first call to our office please have the following information available.
  • Insurance Coverage: Policyholder, ID or Social Security # of insured, Date of Birth of Insured and Employer Group information
  • Medical concerns that may require Pre-Medication prior to your visit (Antibiotics administered)
  • Calendar of available dates for appointments
  • Completed Forms printed from our website (if you have printing capabilites).  If you prefer to complete forms at our office please plan on arriving 15 minutes prior to your scheduled time. We also can mail our office brochure and forms to your residence if time permits prior to your appointment - Just Ask.
